Legal Entity: These services are provided by SDM Medical PLLC, doing business as Stacey Denise Moore, MD, Dr. Stacey Denise, Dr. Stacey Denise | The Neuroaesthetic MD, and The Neuroaesthetic MD. The practice is owned and operated by Stacey Denise Moore, MD.

Educational programs and courses

Community membership and group support

Digital content and resources

Clinical Services provided by SDM Medical PLLC are governed by separate medical agreements and policies and are subject to state-specific medical licensing requirements.

All content, including but not limited to:

Course materials, videos, worksheets, and frameworks

Brand elements, logos, and design

Podcasts, blog posts, and social media content

...are the exclusive property of SDM Medical PLLC and/or Ceyise Studios and are protected by copyright, trademark, and other intellectual property laws.

You May:

Use materials for personal, non-commercial purposes

Share content with proper attribution and a link back to the original source

You May Not:

Reproduce, distribute, or sell program materials

Claim ownership or authorship of our content

Use our trademarks without written permission

Scrape, copy, or repurpose content for AI training or commercial use

Create derivative works or competing programs using our frameworks

Record, screenshot, or redistribute paid program content

4. User-Generated Content

When you participate in our community or submit content (including testimonials, photos, videos, or posts):

You retain ownership of your content

You grant us a perpetual, worldwide, royalty-free, non-exclusive license to use, display, modify, and share your contributions across our marketing and educational platforms

You represent that your content does not violate others' rights or any applicable laws

We reserve the right to remove content that violates our Community Guidelines without notice

You waive any right to inspect or approve the finished product or any marketing materials that include your content

5. Community Guidelines (Facebook & Private Programs)

Our community spaces are designed to be safe, affirming, and supportive. We do not tolerate:

Hate speech, discrimination, or harassment

Medical advice from non-licensed participants

Spam, solicitation, or promotional content

Sharing private information about others

Disruptive or harmful behavior

Impersonation or false representation

Promotion of disordered eating, self-harm, or dangerous health practices

Violations may result in removal from the community without refund and may result in termination of access to all programs and services.

6. Disclaimer of Medical Advice

For Non-Clinical Services:
Coaching, educational programs, and community content are not medical advice, diagnosis, or treatment. They are intended for educational and wellness purposes only. When providing coaching services, Dr. Stacey Denise is acting in her capacity as a health coach, not as a physician. Always consult with a licensed healthcare provider before making health decisions.

For Clinical Services:
Medical care provided by Stacey Denise Moore, MD through SDM Medical PLLC is governed by separate informed consent and telehealth agreements.

Important: If you are participating in both clinical care and coaching programs, you understand that these are separate services with different standards and agreements.

7. No Guaranteed Results

Participation in programs or services does not guarantee specific outcomes. Individual results vary based on adherence, personal circumstances, existing health conditions, and health status. Testimonials and case studies represent individual experiences and are not guarantees of your results.

8. Limitation of Liability

To the fullest extent permitted by law:

We are not liable for indirect, incidental, punitive, or consequential damages

Our total liability is limited to the amount you paid for the service in the twelve (12) months preceding the claim

We are not responsible for technical issues, data loss, or unauthorized access to your account

We are not liable for any actions taken or not taken based on information provided in non-clinical programs or content

We are not responsible for third-party platforms (Skool, Zoom, etc.) or their data practices

9. Indemnification

You agree to indemnify and hold harmless SDM Medical PLLC, Stacey Denise Moore, MD, Ceyise Studios, and their respective officers, employees, contractors, and affiliates from any claims, damages, losses, liabilities, and expenses (including reasonable attorneys' fees) arising from your use of our services or violation of these Terms.

10. Modifications

We reserve the right to modify these Terms at any time. Changes will be posted on this page with an updated "Last Updated" date. Material changes will be communicated via email to active program participants. Continued use of our services after notification constitutes acceptance of modified Terms.

11. Governing Law & Dispute Resolution

Governing Law:
These Terms are governed by the laws of the State of Texas, without regard to conflict of law principles.

Dispute Resolution:
Any disputes will be resolved in the courts of Harris County, Texas. You agree to waive any right to a jury trial.

Arbitration Clause:
Any dispute arising from these Terms shall first be submitted to binding arbitration in Harris County, Texas under the rules of the American Arbitration Association. Each party shall bear their own costs.

12. Severability

If any provision of these Terms is found to be unenforceable or invalid, that provision will be limited or eliminated to the minimum extent necessary so that the remaining Terms remain in full force and effect.

13. Entire Agreement

These Terms, along with our Privacy Policy and any program-specific agreements, constitute the entire agreement between you and SDM Medical PLLC regarding the use of our non-clinical services.

14. SMS/Mobile Messaging Terms

By consenting to Dr. Stacey Denise’s SMS/text messaging service, you agree to receive recurring SMS/text messages from and on behalf of Dr. Stacey Denise through your wireless provider to the mobile number you provided, even if your mobile number is registered on any state or federal Do Not Call list. Text messages may be sent using an automatic telephone dialing system or other technology. Service-related messages may include updates, alerts, and information (e.g., order updates, account alerts, etc.). Promotional messages may include promotions, specials, and other marketing offers (e.g., cart reminders).

You understand that you do not have to sign up for this program in order to make any purchases, and your consent is not a condition of any purchase with Dr. Stacey Denise. Your participation in this program is completely voluntary.

Message and Data Rates: We do not charge for the Service, but you are responsible for all charges and fees associated with text messaging imposed by your wireless provider. Message frequency varies. Message and data rates may apply. Check your mobile plan and contact your wireless provider for details.

Opt-Out: You may opt-out of the Service at any time. Text the single keyword command STOP to 832-299-5229 or reply STOP to any text message to cancel. You'll receive a one-time opt-out confirmation text message. No further messages will be sent to your mobile device, unless initiated by you.
