SDM Medical PLLC provides telemedicine services to patients located in Texas and Ohio. This page explains what telehealth is, how it works, and your rights and responsibilities.
What Is Telehealth?
Telehealth (also called telemedicine) is the delivery of healthcare services using electronic communication technologies, such as video conferencing, secure messaging, and remote monitoring devices.​
Services Provided via Telehealth
- Initial consultations (75–90 minutes)
- Follow-up visits (2-week, 4-week, 8-week check-ins)
- Hormone replacement therapy (HRT) management
- Metabolic health support (including GLP-1 management)
- Nutritional genomics-informed care
- Lab review and interpretation
- Prescription management
- Lifestyle medicine coaching (clinical tier)
- Remote monitoring with wearable devices (optional)
Patient Eligibility & Requirements
You Are Eligible If:
- You are physically located in Texas or Ohio at the time of each visit​
- You have reliable internet access and a device with camera and microphone
- You are able to provide informed consent
- Your condition is appropriate for telehealth evaluation
- You are 18 years of age or older
You Are Not Eligible If:
- You require emergency or urgent care
- Your condition requires in-person physical examination
- You are located outside Texas or Ohio during the visit
- You are unable to participate in video visits
- You are seeking controlled substance prescriptions for initial treatment (restrictions apply per state law)
Technology Requirements
- Computer, tablet, or smartphone with camera and microphone
- Reliable high-speed internet connection (minimum 3 Mbps recommended)
- Updated web browser or telehealth app
- Private, quiet location for appointments
- Backup phone number in case of technical issues
- Email address for receiving appointment links and follow-up information
Supported Platforms: We use HIPAA-compliant telehealth platforms. You will receive connection instructions before your appointment.
Standard of Care
Telehealth services meet the same standard of care as in-person visits. Dr. Stacey Denise Moore will conduct thorough evaluations, review relevant medical history and labs, and develop appropriate treatment plans.​
Good Faith Exam: Texas and Ohio law require that a physician establish a valid physician-patient relationship before prescribing medications via telehealth. This includes an appropriate medical evaluation based on your presenting condition.​
Limitations of Telehealth
- Physical examination is limited to what can be observed via video
- Technical issues may interrupt or delay care
- Not appropriate for emergencies or acute conditions requiring immediate intervention
- Certain procedures cannot be performed remotely
- Prescriptions may have limitations based on telehealth regulations
- Lab work requires in-person collection at a local facility
Emergency Protocol
Telehealth is not for emergencies. If you experience a medical emergency:
- Call 911 immediately
- Go to the nearest emergency room
- Contact your local emergency services
Examples of emergencies include:
- Chest pain or difficulty breathing
- Severe allergic reactions
- Loss of consciousness
- Severe bleeding
- Suicidal or homicidal thoughts
- Stroke symptoms
Privacy & Security
- All video visits use HIPAA-compliant, encrypted platforms
- Your health information is protected according to our Privacy Policy
- Sessions are conducted through secure, password-protected connections
- You are responsible for ensuring your device and internet connection are secure
- Do not share your login credentials with others
- Ensure you are in a private location where others cannot overhear your consultation
Recording Policy: Sessions are not recorded unless you provide explicit written consent. If recording is necessary for clinical documentation, you will be notified in advance.
Informed Consent
Before receiving telehealth services, you will be asked to sign a Telehealth Informed Consent Form acknowledging:
- You understand the nature of telehealth and how it differs from in-person care
- You understand the benefits and limitations of telehealth
- You consent to treatment via telehealth
- You understand the technology requirements and privacy considerations
- You agree to be physically located in Texas or Ohio during all visits
- You understand the emergency protocol and limitations
You may withdraw consent for telehealth services at any time by notifying us in writing.
Prescribing via Telehealth
Dr. Stacey Denise Moore may prescribe medications via telehealth when clinically appropriate. Texas and Ohio regulations require:​
- Establishment of a valid physician-patient relationship
- Appropriate medical evaluation based on accepted standards of care
- Ongoing monitoring and follow-up
- Compliance with controlled substance regulations (if applicable)
Prescribing Limitations:
- Some medications may require in-person evaluation
- Controlled substances are subject to additional restrictions
- Prescriptions are sent electronically to your preferred pharmacy
- You are responsible for pharmacy costs and insurance coverage
Patient Responsibilities
You agree to:
- Be physically located in Texas or Ohio during all visits (you may be asked to verify your location)
- Provide accurate and complete health information, including current medications and medical history
- Follow treatment recommendations and participate actively in your care
- Attend scheduled follow-up appointments
- Pay for services according to our Financial Policy
- Notify us of any changes in your health status or contact information
- Complete any required lab work or testing in a timely manner
- Inform us if you change your physical location or state of residence
Failure to comply with patient responsibilities may result in discontinuation of care.
Insurance & Payment
SDM Medical PLLC is a cash-pay practice. We do not bill insurance directly.
What This Means:
- Payment is due at the time of service
- You will receive a superbill (detailed receipt with diagnosis and procedure codes)
- You may submit the superbill to your insurance company for potential out-of-network reimbursement
- Reimbursement is not guaranteed and is between you and your insurance company
Labs and Medications: May be processed through your insurance when applicable. You are responsible for verifying coverage and paying any copays or deductibles.

Technical Difficulties
If you experience technical difficulties during your appointment:
- Try refreshing your browser or reconnecting to the video platform
- Call our backup number immediately: 832-299-5229
- We will attempt to reconnect via phone or reschedule if necessary
- You will not be charged for appointments that cannot be completed due to technical issues on our end
If you are more than 15 minutes late due to technical issues, your appointment may need to be rescheduled.
State-Specific Requirements
Texas Patients
- Must be physically located in Texas at time of service
- Dr. Moore holds an active Texas medical license (#M8193)
- All care complies with Texas Medical Board telehealth regulations
Ohio Patients
- Must be physically located in Ohio at time of service
- Dr. Moore holds an active Ohio medical license (#35.154137)
- All care complies with Ohio State Medical Board telehealth regulations
