Right to Amend: You have the right to request that we amend your health information if you believe it is incorrect or incomplete. Requests must be made in writing and include a reason.
Right to an Accounting: You have the right to request a list of certain disclosures we have made of your health information in the past six years.
Right to Request Restrictions: You have the right to request restrictions on how we use or disclose your health information. We are not required to agree to your request.
Right to Request Confidential Communications: You may request that we communicate with you in a specific way or at a specific location.
Right to a Paper Copy: You have the right to receive a paper copy of this Notice upon request.
Right to File a Complaint: If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the Department of Health and Human Services. You will not be retaliated against for filing a complaint.
Telehealth-Specific Privacy Considerations
When you receive care via telehealth:
- Video and audio sessions are transmitted using encrypted, HIPAA-compliant platforms
- You must be physically located in Texas or Ohio at the time of service
- Session recordings are not made unless you provide explicit consent
- Your device and internet connection security are your responsibility

We reserve the right to change this Notice. Any changes will apply to health information we already have and to information we receive in the future. The current Notice will be posted on our website and available upon request.
